Instructions for Change Request of Mobile Number/Email ID

  • Fields marked with asterisk  are mandatory
  • OTR number is mandatory for change request
  • Existing mobile number is mandatory for change request
  • Existing mobile number and new mobile number should not be same
  • Existing email ID is mandatory for change request
  • Existing email ID and new email ID should not be same
  • Generate Request Letter before final submission of request
  • Uploading of Request Letter is mandatory
  • Please note that you can file maximum 3 request
  • Make sure that all the details entered are correct.
